Understanding of Liability: I have received or will obtain a copy of Elemental Taekwondo's policies and will take the responsibility to carefully read and follow the rules and policies therein. I understand that Elemental Taekwondo does not give credit and/or refunds for classes missed due to holiday, vacation, illness, weather, etc. I further understand that there are specific risks of physical or property damages, losses, or injury that may result from my or my child's participation with Elemental Taekwondo, and I voluntarily assume the risks associated with such participation.

Photo Release: Elemental Taekwondo is hereby granted permission to take photographs of the students to use in brochures, web sites, advertisements and other promotional materials the school creates. Permission is also hereby granted for the school to copyright such photographs in its name.
